Instructions on Control Testing

-Find a box in fridge that says Coagucheck PT Controls - Take out a vial, pipette, and code chip

-Take Coagucheck machine and put in new code chip

-Login —> Control Test —-> Control Test —> Insert regular test strip —> select the new number for the code chip—> See vial for level/control

-Cut pipette at the end and add water to the vial. Close bottle and swirl until dissolved.

-Add solution same way you would a patient sample

-Replace the code chip and put testing chip back in box

Cleaning Instructions

Put on gloves and use the Super Sani-cloths (see above). Use gloves and ring out moisture if cloths are too wet. Clean inside the test strip guide as well. Operators manual recommends at least monthly cleaning.

Therapeutic Substitution & Adaptation

Therapeutic Substitution: Change of drug (not necessarily within same class). Must be used to achieve same therapeutic goal. Can’t extend duration of original Rx. Can do a therapeutic sub at the same time as an extension.

Examples: Changing antibiotic due to allergy. Switching ACE inhibitor because of a cough. Changing PPI for coverage. Drug shortage needing replacement.

Funded for: drug shortages, better patient outcomes, intolerance/allergy, coverage/cost issues for all patients with NS health card.

Billing MSI: Consent for Therapeutic Sub must be documented.

Adaptation: Same drug but change in dose, frequency, formulation, duration, dosage form. Can’t increase the dose or duration for controlled substances

Examples: Renal dosing, weight based dosing, changing warfarin dose based on INR.

Funded for changes to dose or duration changes that improve clinical effectiives/outcomes for all patients with a NS health card.

Not Funded for: Changing a 5mg tablet to half of a 10mg. Changing qty for blister packs. Change in formulation (tablet to liquid). Change from morning dosing to HS.

Billing MSI: Consent for Adaptation must be documented.
